Демонстрация трех клинических наблюдений иммунной тромбоцитопенической пурпуры (ИТП), развившейся в период пандемии новой коронавирусной инфекции (COVID-19), из личной практики авторов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Представлен краткий обзор литературы, посвященный диагностике и дифференциальной диагностике ИТП, ассоциированной с COVID-19. Приведены три клинических наблюдения данной патологии.</p></sec><sec><title>Результаты</title><p>Результаты. В двух ситуациях тромбоцитопения развилась после перенесенной тяжелой пневмонии, ассоциированной с COVID-19. В одном случае ИТП была диагностирована после перенесенной вирусной инфекции, ПЦР исследование на COVID-19 – отрицательное. У пациента молодого возраста без сопутствующей патологии, с тяжелой тромбоцитопенией и геморрагическим синдромом проводилась терапия глюкокортикоидами и агонистами рецепторов тромбопоэтина (АРТ). После достижения ремиссии сначала были отменены глюкокортикоиды, а потом АРТ. У пожилых пациентов с серьезной коморбидной патологией глюкококртикоиды назначались кратковременно, ремиссия была достигнута при применении АРТ.</p></sec><sec><title>Заключение</title><p>Заключение. Выбор терапии ИТП, развившейся в период пандемии COVID-19, зависит от клинической ситуации, наличия или отсутствия инфекции COVID-19, соматического статуса пациента, наличия коморбидной патологии. АРТ играют важное значение в терапии ИТП, в том числе и ассоциированной с COVID-19.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Demonstration of three clinical cases of immune thrombocytopenic purpura (ITP) that developed during the pandemic of a novel coronavirus infection (COVID-19) from the personal practice of the authors.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A brief review of the literature on the diagnosis and differential diagnosis of COVID-19-associated ITP is presented. Three clinical observations of this pathology are given.</p></sec><sec><title>Results</title><p>Results. In two situations, thrombocytopenia developed after severe COVID-19-associated pneumonia. In one case, ITP was diagnosed after a viral infection; PCR test for COVID19 was negative. A young patient without comorbidities with severe thrombocytopenia and hemorrhagic syndrome was treated with glucocorticoids and thrombopoietin receptor agonists (TPO-RAs). After achieving remission, glucocorticoids were first discontinued, and then TPO-RAs. In elderly patients with a serious comorbid pathology, glucocorticoids were prescribed for a short time, and remission was achieved with TPO-RAs.</p></sec><sec><title>Conclusion</title><p>Conclusion. The choice of therapy for ITP that developed during the COVID-19 pandemic depends on the clinical situation, the presence or absence of COVID-19 infection, the patient’s somatic status, and the presence of comorbid pathology.
