Демонстрация клинического случая пациента с мелкоклеточным нейроэндокринным раком зоны бифуркации трахеи, с метастазами в костный мозг, печень, селезёнку.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Представлен краткий обзор литературы, посвященный диагностике нейроэндокринных опухолей легких и бронхов, и клиническое наблюдение мелкоклеточного нейроэндокринного рака зоны бифуркации трахеи, с метастазами в костный мозг, печень и селезёнку из личной практики авторов.</p></sec><sec><title>Результаты</title><p>Результаты. В клинической картине у данного пациента преобладали «гематологические» проявления: тяжелые тромбоцитопения и геморрагический синдром, анемия, из-за чего вначале проводилась дифференциальная диагностика с системным заболеванием крови. Характерного для метастатического поражения болевого синдрома в костях не было. В постановке диагноза помогли гистологическое исследование биоптата опухоли бронха и трепанобиоптата костного мозга с иммуногистохимическим исследованием (ИГХ), где в обоих случаях при ИГХ были выявлены опухолевые клетки с одинаковым фенотипом.</p></sec><sec><title>Заключение</title><p>Заключение. Метастазирование опухолей в костный мозг может протекать без характерных оссалгий и проявляться только гематологическими синдромами (изменениями в анализах крови). В диагностике помогает цитологическое и гистологическое исследования с выполнением ИГХ, исследования костного мозга.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Demonstration of a clinical case of a patient with small cell neuroendocrine cancer of the tracheal bifurcation zone, with metastases to the bone marrow, liver, spleen.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A brief review of the literature on the diagnosis of neuroendocrine tumors of the lungs and bronchi is presented, and a clinical observation of small cell neuroendocrine cancer of the tracheal bifurcation zone with metastases to the bone marrow, liver and spleen from the personal practice of the authors is presented.</p></sec><sec><title>Results</title><p>Results. The clinical picture of this patient was dominated by “hematological” manifestations: severe thrombocytopenia and hemorrhagic syndrome, anemia, due to which differential diagnosis with a systemic blood disease was initially carried out. There was no pain syndrome in the bones characteristic of metastatic lesions. Histological examination of the bronchial tumor biopsy and bone marrow trephine biopsy with immunohistochemical examination (IHC) helped in making the diagnosis. In both cases, IHC revealed tumor cells with the same phenotype.</p></sec><sec><title>Conclusion</title><p>Conclusion. Metastasis of tumors to the bone marrow can proceed without characteristic ossalgia and manifest itself only in hematological syndromes (changes in blood tests).
