Бронхиальная астма (БА) характеризуется гетерогенностью, множеством фенотипов и различными клиническими проявлениями. Важную роль в воспалительной реакции при бронхиальной астме играют цитокины. Количество, а также соотношение определенных цитокинов определяет механизм и тип воспалительного ответа при бронхиальной астме, от которого зависит эффективность лечения данного заболевания. Именно поэтому требуется разработка новых методов терапии больных с бронхиальной астмой, нацеленных на коррекцию цитокинового дисбаланса. Одним из перспективных и малоизученных веществ является N-эйкозапентаеноилэтаноламин (NAE EPA), который проявляет противовоспалительные свойства, оказывая воздействие на цитокины.</p></sec><sec><title>Цель</title><p>Цель. В условиях in vitro изучить дозозависимый эффект NAE EPA на выработку цитокинов клетками периферической крови у лиц с бронхиальной астмой.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Объектом исследования явилась цельная кровь, разбавленная в соотношении 1:5 культуральной средой 15 пациентов с БА легкой и средней степени тяжести контролируемого течения и 16 здоровых лиц. Эксперимент in vitro проводили в стимулированной липополисахаридом (LPS) разведенной крови (инкубация с LPS при 37°С в течение 30 минут). Затем вносили экспериментальное вещество NAE EPA в концентрациях 1,0; 5,0 и 10,0 мкмоль/л и инкубировали при 37°С ещё в течение 6 часов в режиме плавного перемешивания. Уровни цитокинов: интерлейкинов (IL) (2, 4, 6, 10, 17A), фактора некроза опухоли (TNF)-α и интерферона (IFN)-γ исследовали методом иммуноферментного анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Анализ уровня цитокинов у больных БА показал, что повышение содержания в плазме крови IL-2, TNFα, IL-6, IL-17A сопровождалось снижением уровня регуляторного IL-10. При внесении NAE EPA в дозировке 1 мкмоль/л статистически значимых изменений не было выявлено. Воздействие экспериментального вещества в дозе 5 мкмоль/л способствовало снижению содержания IL-6 в клетках крови больных на 19% (p ˂ 0,05). При воздействии NAE EPA в дозировке 10 мкмоль/л наблюдалось наибольшее количество статистически значимых изменений в уровнях цитокинов. Выявлялось снижение уровня IL-17A на 15% (p ˂ 0,05), IL-2 – на 14% (p ˂ 0,05), IL-6 – на 50% (p ˂ 0,01), TNF-α – на 10% (p ˂ 0,05) относительно значений до воздействия.</p></sec><sec><title>Заключение</title><p>Заключение. NAE EPA демонстрирует потенциал как регулятор синтеза про- и противовоспалительных цитокинов при бронхиальной астме с превалированием Th-17 типа иммунного ответа. Полученные результаты могут быть использованы в разработке новых стратегий лечения больных с бронхиальной астмой.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Bronchial asthma is characterized by heterogeneity, multiple phenotypes, and varying clinical manifestations. Cytokines play a crucial role in the inflammatory response in asthma. The quantity, as well as the ratio of certain cytokines, determines the mechanism and type of inflammatory response in asthma, upon which the effectiveness of treatment of this disease depends. That is why the development of new methods of treating patients with asthma, aimed at correcting cytokine imbalance, is required. One of the promising substances is N-eicosapentaenoyl-ethanolamine (NAE-EPA), which exhibits anti-inflammatory properties by affecting cytokines, but remains poorly studied.</p></sec><sec><title>Aim</title><p>Aim. To study the dose-dependent effect of N-eicosapentaenoyl-ethanolamine on the production of cytokines by peripheral blood cells, in vitro, in subjects with asthma.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The object of the study was whole blood, diluted 1:5 with culture medium of 15 patients with mild to moderate controlled asthma and 16 healthy subjects. The in vitro experiment was carried out in lipopolysaccharide-stimulated (LPS) blood samples (incubation with LPS at 37°C for 30 minutes). Then, the experimental substance N-acylethanolamine eicosapentaenoic acid (NAE EPA) was added in concentrations of 1.0; 5.0, and 10.0 µM and incubated at 37°C for 6 hours with gentle mixing. Cytokine levels (IL-2, IL-4, IL-6, IL-10, IL17A, TNF-α, and INF-γ) were studied by enzyme-linked immunoassay.</p></sec><sec><title>Results</title><p>Results. Analysis of the level of cytokines in patients with asthma showed that an increase in the plasma levels of IL-2, TNF-α, IL-6, and IL-17A is accompanied by a decrease in the level of regulatory IL-10. When NAE EPA was added at a dosage of 1 µM, no statistically significant changes were detected. Exposure to the experimental substance at a dose of 5 µM contributed to a decrease in IL-6 in the blood cells of patients by 19% (p ˂ 0.05). Exposure to NAE EPA at 10 µM produced the greatest number of statistically significant changes in cytokine levels. There was a decrease in IL-17A by 15% (p ˂ 0.05), IL-2 by 14% (p ˂ 0.05), IL-6 by 50% (p ˂ 0.01), and TNF-α by 10% (p ˂ 0.05) relative to values before exposure.</p></sec><sec><title>Conclusion</title><p>Conclusion. N-eicosapentaenoyl ethanolamine shows potential as a regulator of pro- and anti-inflammatory cytokine synthesis in bronchial asthma with a predominant Th-17 type of immune response.
