Оценить возможности прогнозирования риска частых обострений у пациентов с хронической обструктивной болезнью легких (ХОБЛ) с использованием показателей функционального состояния кардиореспираторной системы.Материалы и методы. В одногодичное проспективное одноцентровое исследование было включено 85 пациентов с обострением ХОБЛ, из которых по результатам наблюдения в течение года были сформированы две группы. В 1 группу вошли пациенты (n = 28) с одним обострением или отсутствием обострений за год, во 2 группу – пациенты (n = 57) с 2 и более обострениями в течение года, имевшие более выраженные нарушения функции внешнего дыхания и сердечной деятельности. Комплексное клинико-функциональное обследование включало заполнение вопросников САТ, mMRC, КОП-25, исследование функции внешнего дыхания и эхокардиографическое исследование.Результаты. По данным дискриминантного анализа были созданы две модели прогнозирования с высокой эффективностью позволяющие выделять пациентов с повышенным риском частых обострений в течение года. В качестве предикторов использовались результаты оценки по вопросникам, показатели внешнего дыхания (объем форсированного выдоха за 1 секунду, максимальная объемная скорость выдоха на уровне 50%) и эхокардиографические параметры (толщина межжелудочковой перегородки и смещение трикуспидального кольца в систолу – TAPSE).Заключение. Полученные модели могут быть использованы для разработки технологии прогнозирования неблагоприятного течения ХОБЛ с целью обеспечения персонализированного подхода при выборе лечения пациентов с высоким риском развития обострений.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To study the possibility of predicting frequent exacerbations of chronic obstructive pulmonary disease (COPD) by assessing parameters of lung function and echocardiographic indices.Materials and methods. A oneyear prospective single-center study included 85 patients with COPD exacerbations, of whom two groups were formed based on the results of observation during the year. Group 1 included patients (n = 28) with one or no exacerbations per year, group 2 included patients (n = 57) with two or more exacerbations during the year (frequent exacerbations). It was found that patients in group 2 had more pronounced impairments of lung and cardiac function. A comprehensive clinical and functional examination included the CAT, mMRC, KOP-25 questionnaires, a study of lung function and an echocardiographic examination.Results. Based on discriminant analysis data, two highly effective prediction models were developed to identify patients at increased risk of frequent exacerbations over the course of a year. Questionnaire results, respiratory parameters (FEV1, FEF50), and echocardiographic parameters (interventricular septal thickness and tricuspid annular systolic displacement (TAPSE)) were used as predictors.Conclusion.
