Коморбидные состояния при хронической обструктивной болезни легких (ХОБЛ) могут существенно ухудшить качество жизни и прогноз, способствовать частым обострениям и увеличивать риск госпитализации.</p></sec><sec><title>   Цель</title><p>   Цель. Изучить влияние коморбидных состояний на сердечную деятельность у пациентов с обострением ХОБЛ.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. В исследование включены 103 пациента с обострением ХОБЛ, распределенных в две группы. Первую группу составили 36 больных ХОБЛ без коморбидных состояний, вторую группу – 67 больных с коморбидным фоном. В контрольную группу включены 25 человек без ХОБЛ. Повторные исследования проведены через 3–6 и 9–12 месяцев. Функцию внешнего дыхания оценивали с помощью спирометрии форсированного выдоха до и после пробы с бронхолитиком, бодиплетизмографии, измерения диффузионной способности легких методом одиночного вдоха СО. В группах больных оценивали степень выраженности одышки по шкале mMRC и качество жизни по вопроснику САТ. В плазме крови определяли уровень NT-proBNP методом иммуноферментного анализа. Оценку структурно-функционального состояния сердца проводили посредством эхокардиографии секторальным датчиком 5 МSс c частотой 1,5–4,6 мГц с оценкой глобальной продольной деформацией желудочков с использованием спекл-трекинга.</p></sec><sec><title>   Результаты</title><p>   Результаты. В обеих группах больных при обострении ХОБЛ диагностированы изменения сердечной деятельности со снижением систолической и диастолической функций обоих желудочков, наиболее выраженные в группе больных ХОБЛ с коморбидными состояниями. По данным корреляционного анализа эти нарушения тесно связаны со степенью бронхиальной обструкции и нарушениями диффузионной способности легких, сопряжены с увеличением уровня NT-proBNP. У больных группы без коморбидного фона отмечалась частичная коррекция ряда эхокардиографических показателей через 3–6 месяцев после обострения.</p></sec><sec><title>   Заключение</title><p>   Заключение. Обострение ХОБЛ сопровождается значительными изменениями сердечной деятельности, степень выраженности которых существенно зависит от наличия коморбидных состояний. Результаты анализа подтверждают необходимость активного мониторинга сердечной деятельности у пациентов с ХОБЛ в качестве основы для разработки терапевтических стратегий, направленных на улучшение клинических исходов и снижение бремени коморбидных состояний.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Introduction</title><p>   Introduction. Comorbid conditions in chronic obstructive pulmonary disease (COPD) can significantly worsen quality of life and prognosis, contribute to frequent exacerbations, and increase the risk of hospitalization.</p></sec><sec><title>   Aim</title><p>   Aim. To study the effect of comorbid conditions on cardiac performance in patients with COPD exacerbation.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. The study included 103 patients with COPD exacerbation, divided into two groups. The first group consisted of 36 patients without comorbid conditions, the second group – 67 patients with comorbidities. The control group included 25 persons without COPD. Repeated studies were conducted after 3–6 and 9–12 months. Lung function was assessed using spirometry before and after the bronchodilatory test, bodyplethysmography, and measuring the lung diffusion capacity using the CO single breath technique. The severity of dyspnea was assessed using the mMRC scale and quality of life using the CAT questionnaire. NT-proBNP levels were determined in blood plasma using immunochromatography. The structural and functional state of the heart was assessed using echocardiography with a 5 ms sector sensor at a frequency of 1.5–4.6 MHz with an assessment of global longitudinal strain of the ventricles using speckle tracking.</p></sec><sec><title>   Results</title><p>   Results. In both groups of patients during an exacerbation of COPD, we diagnosed changes in cardiac performance with a decrease in systolic and diastolic functions of both ventricles, which were most pronounced in the group of COPD patients with a comorbid background. According to the correlation analysis, these disorders are closely associated with the degree of bronchial obstruction and impaired diffusion lung capacity, associated with an increase in NT-proBNP levels. In the group without a comorbid background, partial correction of a number of echocardiographic indices was noted 3–6 months after the exacerbation.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. COPD exacerbations lead to significant changes in cardiac function, the severity of which significantly depends on the presence of comorbidity.
