Множественная миелома (ММ) является одним из самых распространенных гемобластозов.</p></sec><sec><title>Цель</title><p>Цель. Демонстрация возможностей применения таргетного моноклонального антитела – даратумумаб у пациентов с впервые выявленной ММ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Приведены два собственных клинических наблюдения, демонстрирующих эффективность и безопасность режимов на основе даратумумаба у пациентов с впервые выявленной ММ.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты получили лечение в первой линии с включением даратумумаба. У первого пациента терапия была начата по протоколу Dara-Vd (даратумумаб, бортезомиб, дексаметазон), у второй пациентки – Dara-Rd (даратумумаб, леналидомид, дексаметазон). Выбор схемы определялся исходным статусом и наличием остеодеструктивного синдрома. У обоих был достигнут частичный ответ, после чего выполнена аутологичная трансплантация стволовых гемопоэтических клеток (ауто-ТСГК), а в дальнейшем проведено консолидирующее лечение с использованием даратумумаб-содержащих протоколов. После чего была достигнута полная ремиссия ММ.</p></sec><sec><title>Заключение</title><p>Заключение. Представленные клинические случаи иллюстрируют высокую эффективность даратумумаба в первой линии ММ у кандидатов на ауто-ТСГК, возможность достижения ответа после 3–6 циклов терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Multiple myeloma (MM) is one of the most common hematologic malignancies.</p></sec><sec><title>Aim</title><p>Aim. To demonstrate the potential of using the targeted monoclonal antibody daratumumab in patients with newly diagnosed MM.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Two clinical cases illustrating the efficacy and safety of daratumumab-based regimens in newly diagnosed MM are presented.</p></sec><sec><title>Results</title><p>Results. Both patients received first-line therapy including daratumumab. The first patient was treated according to the Dara-Vd protocol (daratumumab, bortezomib, dexamethasone), while the second received Dara-Rd (daratumumab, lenalidomide, dexamethasone). The choice of regimen was guided by baseline disease status and the presence of osteolytic bone lesions. Both patients achieved a partial response, followed by autologous stem cell transplantation (aHSCT). Subsequently, consolidation therapy with daratumumab-containing protocols was administered, resulting in complete remission of MM.</p></sec><sec><title>Conclusion</title><p>Conclusion.
