В статье представлен обзор современной литературы по проблеме ХОБЛ у женщин. Приведены данные о распространенности заболевания, об особенностях чувствительности к воздействию факторов риска, возрастных аспектов развития, прогрессирования, исходов и клинических проявлений ХОБЛ, а также о различии в реагировании на проводимую фармакотерапию у женщин и мужчин. Так, в ряде исследований продемонстрирована высокая восприимчивость женщин к воздействию табачного дыма, предрасположенность к развитию бронхитического фенотипа ХОБЛ, тяжелому течению с частыми обострениями и низким риском смерти после обострений. По результатам проведенного обзора можно заключить, что гендерные различия могут быть объяснены анатомическими особенностями развития дыхательной системы и циклической гормональной активностью женского организма, что проявляется большей выраженностью гиперреактивности дыхательных путей, и, как следствие, высокой частотой тяжелого течения ХОБЛ.</p></abstract><trans-abstract xml:lang="en"><p>Chronic obstructive pulmonary disease (COPD) remains the focus of medical science and practice. The article presents a review of contemporary literature on the problem of COPD in women. The data are presented on the prevalence of the disease, on the characteristics of sensitivity to the effects of risk factors, age-related aspects of development, progression, outcomes and clinical manifestations of COPD, as well as on the difference in response to the pharmacotherapy in women and men. Thus, a number of studies have demonstrated a high susceptibility of women to exposure to tobacco smoke, a predisposition to the development of the bronchitis phenotype of COPD, severe course with frequent exacerbations and a low risk of death after exacerbations. Based on the results of the review, it can be concluded that gender differences can be explained by the anatomical features of the respiratory system development and the cyclic hormonal activity of the female body, which is manifested by a greater degree of airway hyperresponsiveness, and, as a consequence, a high frequency of severe COPD.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>хроническая обструктивная болезнь легких</kwd><kwd>женщины</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Tronic obstructive pulmonary disease</kwd><kwd>women</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Всемирная организация здравоохранения.
