В литературном обзоре рассмотрены современные методы, применяемые в диагностике бронхиальной астмы. В первую очередь, это физикальное обследование пациентов и выяснение анамнестических особенностей, исследование функции внешнего дыхания. На сегодняшний день «золотым стандартом» в диагностике и мониторировании бронхиальной астмы являются спирометрические исследования (спирография, пиклофлоуметрия, бодиплетизмография с применением различных провокационных проб), однако, они полностью не отражают разностороннюю картину заболевания. Для подтверждения диагноза необходима оценка аллергологического статуса, определение биологических маркеров заболевания. Используется ряд методик для изучения нарушения регионарной вентиляции: радиосцинтиграфия, лучевые методы (в первую очередь мультиспиральная компьютерная томография высокого разрешения с трехмерной визуализацией). Одним из перспективных направлений в диагностике являются генетические методы. Следует учитывать вариабельность бронхиальной астмы и применять в диагностике комплексный подход.</p></abstract><trans-abstract xml:lang="en"><p>Bronchial asthma is a serious social and medical problem. This review deals with modern methods used in asthma diagnostics. In the first place it's a physical examination of patients and revealing of anamnestic characteristics as well as the study of respiratory function. Today spirometry studies are a «gold standard» for diagnostics and monitoring of asthma (spirography, peak flowmetry, body plethysmography with various provocative tests), but they don't fully reflect the multifaceted disease picture. Allergic status evaluation and identification of biological markers of asthma are necessary. There are a number of technical methods used to study disorders of regional ventilation: scintillation scanning, X-ray methods (multidetector computed tomography with three-dimensional-imaging). Modern genetic methods are one of the future trends in asthma diagnostics.
