Произведено изучение летальности от венозных тромбоэмболических осложнений за три года по результатам патологоанатомических исследований 1764 пациентов. При исследовании учитывали возраст, пол, время года, основное заболевание, по поводу которого пациент находился в стационаре, сроки смерти с момента поступления в стационар, источник первичного тромбообразования, изменения в легочной ткани, уровень обтурации легочного венозного русла эмбологенными массами. Установлено, что у 5,4% больных причиной смерти явились венозные тромбоэмболические осложнения - 0,8 на 1000 населения в год. Средний возраст пациентов, погибших от тромбоэмболии легочной артерии - 58 лет, по половому признаку преобладали женщины - 58%. Структура патологии, по поводу которой пациенты получали стационарное лечение: послеоперационный период после многопрофильных хирургических вмешательств - 31%, острые нарушения мозгового кровообращения - 32%, многопрофильная терапевтическая патология - 37%. У 20% смерть наступила от массивной тромбоэмболии легочной артерии в первые сутки с момента поступления, 25% погибли через 2-7 дней, 57% - от 7 дней до нескольких месяцев. Источником первичного тромбообразования у 52% пациентов явились венозные синусы голени, у 22% - подколенно-бедренно-подвздошный сегмент и нижняя полая вена, у 10% - правые отделы сердца, у 16% пациентов источник не обнаружен. У 2/3 больных имелась острая обтурация легочного ствола и главных ветвей легочной артерии. Разработаны способы предупреждения развития венозных тромбоэмболических осложнений и массивной тромбоэмболии легочной артерии у стационарных больных Амурской области.
The study of lethality from venous thrombo-embolic complications was done for three years by the results of pathological-anatomical examinations of 1764 patients. Age, sex, the season of the year, the main disease because of which the patient was in hospital, the dates of death since the moment of the patient getting to hospital, the sources of the initial thrombi formation, the changes in the pulmonary tissue, the level of obturation of pulmonary venous bed by embologenic masses were considered. It was found out that in 5.4% of the patients the cause of the death was venous thrombo-embolic complications: 0.8 per 1000 of population a year. The mean age of patients who died from thromboembolism of pulmonary artery is 58 years, by the sex parameter women dominated (58%). The structure of pathology because of which patients had the treatment in hospital is the following: postoperative period after multiprofile surgeries (31%), severe disorders of brain blood circulation (32%), multiprofile therapeutic pathology (37%). 20% of patients died from massive thromboembolism of pulmonary artery at the first day after it happened, 25% died in 2-7 days, 57% died at the 7th day or in several months. The source of the first thrombi formation in 52% of patients became venous sinuses of anticnemion, in 22% it was popliteus-thigh-iliac segment and low cava, in 10% these were the right compartments of heart, in 16% the source was not identified. In 2/3 of patients there was an acute obturation of the pulmonary trunk and of main branches of the pulmonary artery.
