АЛП является редким, но потенциально излечимым заболеванием неизвестной этиологии, характеризующимся нарушением метаболизма сурфактанта, что становится причиной накопления в альвеолах белкового материала (поверхностно-активных веществ) и его компонентов, включая фосфолипиды. Особенностью АЛП (а это, безусловно, учитывается в диагностическом процессе), является скудная клиническая картина, не соответствующая обширным легочным поражениям, регистрируемым на рентгенограммах и, особенно, при мультиспиральной компьютерной томографии (МСКТ) с постпроцессинговой обработкой изображений. В данной статье проанализирована отечественная и зарубежная литература последних лет, отражающая процессы диагностики, лечения и реабилитации больных АЛП. За последние 5 лет мы обследовали, лечили и наблюдали в динамике 6 больных АЛП (объединённый реестр Амурской области и Приморского края). В данном сообщении представлено одно из оригинальных наблюдений, где на разных этапах при неоднократных предварительных обследованиях пациентки с АЛП клиническая и КТ-диагностика были ошибочны. Достаточно сказать, что четыре МСКТ- исследования были сделаны в различных подразделениях Амурской области и в Приморском крае с доминирующим заключением - идиопатический фиброзирующий альвеолит, и это несмотря на то, что АЛП имеет свои достаточно специфичные диагностические признаки, существенно отличающиеся от клинико-радиологических признаков идиопатического фиброзирующего альвеолита. По причине клинико-радиологических ошибок была неправильно выполнена первая открытая биопсия без специальной окраски биоптатов на наличие белковых компонентов. В результате системных ошибок проведено несколько курсов неадекватной кортикостероидной терапии, что способствовало возникновению осложнений течения заболевания в виде рецидивирующей полисегментарной пневмонии. Окончательное клиническое обследование позволило заподозрить аутоиммунную форму АЛП, и повторная открытая биопсия с окрашиванием биоптата реактивом Шиффа (PAS-реакция) позволила установить достоверный диагноз. При динамическом мониторинге через 6 месяцев, включающем МСКТ, у данной больной зарегистрирована практически полная спонтанная регрессия патологических измерений в легочной ткани.</p></abstract><trans-abstract xml:lang="en"><p>In standard classifications of interstitial pulmonary diseases alveolar pulmonary proteinosis (APP) by etiologic signs is referred to the group of idiopathic ones, where besides APP there is sarcoidosis, idiopathic pneumofibrosis, Hand-Schuller-Christian disease, idiopathic interstitial alveolitis (acute interstitial pneumonia, Hamman-Rich syndrome) and others. APP is a rare but potentially curable disease of unknown etiology, characterized by the disorder of surfactant metabolism, which leads to accumulation of protein in alveoli (superficially active substances) and its components, including phospholipids. The main feature of APP (and this is definitely taken into account in diagnostics) is its poor clinical picture which does not correspond to large pulmonary damages registered in roentgenograms and at multispiral computer-aided tomography (MSCT) with post processing image processing. This article gives a review of recently published domestic and foreign literature. During 5 years we examined, treated and observed in dynamics 6 patients with APP. This report presents one research whose feature is the wrong clinical diagnosing of the disease at all preliminary stages of examination of the patient with APP. Because of clinical mistakes open biopsy was done in a wrong way, without special painting of biopsy material upon the presence of protein components. As a result of system mistakes several courses of improper therapy were done, which led to complications of the disease in the form of recurrent pneumonia. Final clinical-radiologic examination allowed to diagnose autoimmune form of APP, and the repeat open biopsy with the painting of biopsy material with Schiff's reagent (PAS-reaction) gave us a chance to make a final diagnose.
