Для изучения информативности показателей, определяемых при проведении бодиплетизмографии и определении диффузионной способности легких по оксиду углерода методом одиночного вдоха, было обследовано 493 больных с различными нозологическими формами интерстициальных заболеваний легких. Показано, что по изменению альвеолярного объема (АО) можно судить о площади поверхности газообмена, а объем невентилируемого пространства (∆ОЕЛ = ОЕЛплет - ОЕЛНе) является количественной характеристикой «воздушной ловушки». В зависимости от величин АО и ∆ОЕЛ было выделено четыре синдрома нарушений газообмена: синдром повреждения альвеолярно-капиллярной мембраны, синдром «воздушной ловушки», синдром сокращения поверхности газообмена и синдром смешанных нарушений газообмена. Предложенная концепция интерпретации данных важна для понимания причин нарушений легочного газообмена у больных интерстициальными заболеваниями легких и выявления индивидуальных особенностей течения болезни.</p></abstract><trans-abstract xml:lang="en"><p>The article presents the analysis of possibilities to identify the causes of pulmonary gas exchange abnormalities in patients with interstitial lung diseases. The parameters of single-breath carbon monoxide diffusing capacity (DL,CO) and body plethysmography were analyzed in 493 patients with interstitial lung diseases. The alveolar volume (VA) was informative for the characteristic of gas-exchanging surface and the calculation of the noncommunicating gas (∆TLC = TLCpleth - TLCHe) was informative for the quantitative estimation of «air trapping». There were determined the four syndromes of specific diffusion disturbances depending on the values of VA and ΔTLC: the syndrome of damage to the alveolar-capillary membrane, the syndrome of «air trapping», the syndrome of gas-exchanging surface reduction and the syndrome of mixed gas exchange abnormalities.
